Pharmacy support designed for care homes

Everything your care home needs from its pharmacy

Clear systems. Practical support. Reliable communication.

A good care-home pharmacy service should reduce pressure on your team rather than create more work.

Care Pack Pharmacy provides structured medicines support built around your residents, your medication cycle and the way your team works day to day.

We can support your home with:

• organised repeat medication cycles

• Our safer, colourful removable-pod trays system where appropriate

Original-packs medication supply where safer or more suitable

• clear MAR charts with medication pictures where available

• support for paper MAR charts and a range of eMAR systems

• new medicines, stopped medicines, dose changes, interims and hospital discharges

• a direct route for urgent matters and a monitored email route for routine communication

• planned delivery and clear handover arrangements

Our aim is to give your team a pharmacy relationship that feels organised, responsive and easier to manage.

What your home can expect

MultiMeds removable-pod trays designed for care-home routines

A tray system care-home teams immediately recognise and value

Our MultiMeds system organises suitable routine medicines into clearly labelled removable pods arranged by administration time.

Each pod can be removed individually which can make planned appointments, days out and temporary leave easier to manage in line with the home’s medicines procedures.

Why care-home teams value MultiMeds:

• clear colour coding for different administration times

• individual removable pods

• clear medicine information on each pod

• resident identification with a photograph where used and appropriate

• practical handling during medication rounds

original-pack medicines supplied alongside the tray where required

Each resident’s medication is considered individually. MultiMeds trays are only used where they are suitable following pharmacy review.

Current introductory offer!

Eligible new care homes joining while onboarding spaces remain available can receive MultiMeds tray supply at no additional charge.

Future tray and onboarding arrangements may differ once the current introductory spaces have been filled.

Organised repeat medication cycles

We agree the cycle dates, ordering process and key responsibilities with your team. Missing prescriptions or information are identified and followed up as early as possible to reduce avoidable delays.

Medicine changes and interim supplies

We provide practical support with new medicines, stopped medicines, dose changes, interims, hospital discharges, owings and medication queries.

Clear MAR chart support

Where paper MAR charts are used, we can agree a structured process for preparation, changes, collection and return.

Our MAR charts can include clear presentation and medication pictures where available.

Flexible eMAR support

We work with homes using paper MAR charts and a range of eMAR systems.

Before the service begins, we agree how prescriptions, medicine changes, interims, fridge items, PRN medicines and communication will be handled.

System functionality and integration depend on the eMAR provider and the arrangements agreed with the home.

Communication when you need it

Your team receives:

• a clear route for urgent and time-sensitive matters

• a monitored email route for routine orders, changes and records

• clear ownership of queries and follow-up

• pharmacist escalation where clinical or professional input is required

Planned local support

For homes within our local service area, we can agree routine delivery arrangements and practical support with collections and handovers.

Where agreed and operationally possible, this can include collecting paper MAR charts and returning them promptly once prepared.

Planned monthly cycle delivery across England

Suitable care homes further afield can receive their planned monthly medication cycle through an agreed tracked delivery service.

This may include MultiMeds removable-pod trays and original-pack medicines where appropriate following pharmacy review.

We plan the cycle in advance, agree delivery and handover arrangements and keep your team informed if anything is missing or could delay supply.

Enquiries from care homes elsewhere in the UK can be considered individually because prescription and pharmacy arrangements differ between England, Scotland, Wales and Northern Ireland.

Urgent and acute medicines

Scheduled postal or courier delivery is designed for planned medication cycles rather than treatment that must begin immediately.

If an antibiotic or another acute medicine needs to start without delay, we will work with the home and prescriber so the prescription can be directed to an appropriate local pharmacy or urgent supply route.

Care Pack Pharmacy can continue managing the resident’s planned monthly cycle and support the home with clear communication about how the acute treatment affects the ongoing medicines process.

This provides an organised monthly service without allowing delivery times to delay genuinely urgent treatment.

Getting started with Care Pack Pharmacy

Switching to Care Pack Pharmacy is a clear, supported process

We understand that changing pharmacy can feel disruptive.

Our onboarding process is designed to identify missing information early, agree responsibilities clearly and reduce avoidable problems during the first medication cycle.

1. Understand your home

We review resident numbers, GP links, current cycle dates, tray requirements, original-pack medicines, MAR or eMAR arrangements and the issues you most want improved.

2. Plan the switch

We agree the information required, prescription timescales, delivery arrangements, communication routes and proposed go-live date.

3. Identify missing information early

We prompt your team when prescriptions, resident information, GP details or medicines records are missing so they can be resolved before they delay the first cycle.

4. Support the first cycle

Our team stays close to the home during go-live, checks the agreed arrangements and responds promptly to early queries.

5. Establish smoother ongoing communication

Once the initial setup is complete, the home moves into a clearer repeat-cycle, delivery and communication routine.
